No. Currently, only a valid passport is required for those staying less than 60 days in Thailand.
No vaccinations are required, unless you're planning on backpacking/sleeping in the open jungle.
Shorts (at least mid-thigh length), capris, sundresses/dresses (nothing too short - mid-thigh length or longer), jeans are acceptable (Thailand is very hot so jeans may not be the most comfortable), leggings, jeggings, and yoga pants are acceptable.
No spagehetti strapped shirts or tank tops. Shirts with minimal cap-sleeve are acceptable. (Religous places, like temples, require conservative attire - shoulders, midriffs, and knees should be covered).
Shoes are removed when entering homes and most businesses. Flip flops and sandles are perfectly acceptable - as well as tennis shoes. You'll want to make sure the shoes you pack are easily removable and comfortable.
